Lafayette School District Volunteer Requirements

The Lafayette School District (LafSD) uses "BE A MENTOR," an online portal, to streamline and track volunteer screening. For all volunteer roles at LES, you will be required to have TB Screening Clearance AND Live Scan Fingerprint Background Check Clearance documented in the BE A MENTOR system.

 

Some of the requirements take several weeks to complete or process, so please start NOW!

On Sunday, Aug. 10, 2025, we will have Live Scan fingerprinting and TB Clearance stations on campus during the Leopard Day event from 10:00 am - 12:00 pm. Email volunteers@lafayettepta.info to request a time-slot (depending on demand, walk-ups may also be available). 


Step 1: Create A 'Be A Mentor' Account

All volunteers must register on the 'Be A Mentor' online portal; you'll need to:

  • Select the "Volunteer Chaperone" Activity option
  • Upload a copy or photo of your Drivers Licence 
  • Affirm that you have completed the online training & orientation (by reading and signing the provided clearance form and watching the provided FERPA video)   
  • If you already have a Be A Mentor account, update or confirm your volunteer statusCREATE or UPDATE YOUR ACCOUNT NOW

Once you complete the additional steps below, you will need to upload the appropriate documentation to your 'Be A Mentor' profile.

 

Step 2: Get Fingerprinted Via Live Scan

All volunteers (even one-time volunteers) must pass a Live Scan background check. It can take several weeks for results to come in, so please get this done ASAP. Once your District-Specific Live Scan Clearance status HAS BEEN approved in the Be A Mentor system, you will not need to repeat this step.

• Find a local LIVE SCAN PROVIDER. Many UPS Stores allow for walk-ins, please call before going.

• Fill out THIS FORM and bring to your appointment along with ID. Note: You must bring this exact form to your appointment.  Live Scan results submitted on any other version of the form will be rejected by the school district. Depending on location/vendor used, the appointment may cost $40 to $75+.

• Once you've completed your Live Scan fingerprinting appointment, please allow up to 3-4 weeks for the results to be processed. Processed results are automatically sent to the district and may take another several days to be reflected in your Be A Mentor profile. 

• District-approved/confirmed Live Scan clearance reflected in Be A Mentor does not expire. Check your Be A Mentor status to confirm if you believe this entire step was completed last year.

 

 

Step 3: TB (Tuberculosis) Screening Clearance

All volunteers must show proof of TB clearance within the last four years.

IF YOU DO NOT HAVE TB CLEARANCE FROM WITHIN THE LAST FOUR YEARS:

• You must receive a TB Screen by a healthcare provider. It is a standard screening involving 5 simple questions followed by a signature from your healthcare provider. The results of a TB screen may result in the need for a TB skin test or blood test if certain risk factors are identified.

 

• You can contact your own healthcare provider, or find COMMUNITY TESTING OPTIONS

 

• Most providers provide their own TB Screening form OR you can ask them to FILL OUT THIS ONE

 

IF YOU DO HAVE TB CLEARANCE DOCUMENTATION FROM WITHIN THE LAST FOUR YEARS:

• Simply upload that documentmentation to your 'Be A Mentor' profile and/or check your status to confirm current clearance.

 

 

UPLOAD TO 'BE A MENTOR' NOW 

 

 

Step 4: Upload Declaration Of Insurance*

*This Step is Required Only for Volunteers that WANT TO DrivE STUDENTS for a School Field Trip

• Select the "Volunteer Driver" Activity option in your BE A MENTOR profile.

 

• Upload Your Declaration of Insurance to the 'Be A Mentor' portal!

 

PLEASE NOTE:

- THE DECLARATION OF DRIVERS INSURANCE IS NOT YOUR Insurance CARD. IT IS A PAGE ON YOUR POLICY THAT SHOWS THE NAME OF YOUR INSURANCE COMPANY, YOUR NAME AS AN INSURED DRIVER AND YOUR POLICY EXPIRATION DATE

 

- Most people's Insurance Declaration dates expire/renew twice a year, so this step may need to be repeated or updated for Volunteer Driver status to remain current.

 It is important to the Lafayette School District that all families have access to volunteer opportunities at their child’s school. If the cost of the fingerprinting service is prohibitive, parents/caregivers are encouraged to speak directly with their principal. Additionally, all families of students who qualify for free/reduced lunch, are homeless or foster youth, or are English language learners will be contacted by the District to assist in arranging for free fingerprinting services.
